Emma's Story Emma, an 87 year old widow, who lives alone, had been declining in functioning. Her memory has begun to fail. She is becoming more confused and does not always remember to take her medication. She needs more assistance with bathing and cooking. Emma's daughter turned to Senior Assistance for help. We discussed with her the pros and cons of Emma staying in her own home with help or going to a personal care home. Emma's daughter decided to keep her mother in her home and hired a home health attendant to provide assistance with bathing, cooking and medication reminders. Along with this, Senior Assistance developed a plan of care for the home health aid to perform with Emma. This plan of care included daily discusisons of current events and other activites with Emma to help slow her memory decline. Weather permitting; they were to plan outings that Emma enjoyed. Senior Assistance provided oversight to this plan of care to assure that it was being followed.
